Man killed when vaping pen explodes, sending pieces into his skull

The Pinellas-Pasco Medical Examiner's autopsy report released Tuesday confirms the vape pen was responsible for the May 5 death of Tallmadge D'Elia, 38, in St. Petersburg. His official cause of death was listed as "projectile wound of the head" and ruled accidental.

According to the report, the pen he used was a "mod" type, or a modified e-cigarette created to be customizable.

A former CNBC producer was killed when his e-cigarette exploded and lodged into his skull, penetrating his brain, according to an autopsy, the Tampa Bay Times reports.

And in 2016, a vape pen user in NY suffered third-degree burns after the pen exploded in his trousers, NBC reported. It's unclear what caused the pen to explode.

The explosions usually occur suddenly, the report found, "and are accompanied by loud noise, a flash of light, smoke, flames, and often vigorous ejection of the battery and other parts". These include using vape devices with added safety features, replacing wet or damaged batteries, avoiding charging overnight and using only e-cigarette chargers. Since most e-cigarettes use lithium batteries, there is commonly known high risks associated with improper use of the electronics.
